[DFDL-WG] Scoping Rules with Schema default

I have written up the new rules for scoping in the attached document for discussion on this weeks call. I have also updated and extended Suman's examples. Note that he dfdl properties aren't necessarily correct for the construct and are only used to indicate where they came from. I have not yet extended to multiple schema.
