I updated the references so they are hyperlinks now. Unfortunately, getting MS Word to do this easily required changing them from symbolic refs e.g., [XMLSch] to numeric ones e.g., [7].

I also updated affiliations for contributors where I could find their current affiliations. 3 are still unknown to me: Dave Glick, USA, Tara Talbot, PNNL, and Steve Marting, Progeny, USA. I am unable to find these people by web search or LinkedIn.
